Overactive Thyroid (Hyperthyroidism)
Hyperthyroidism is a disorder in which your thyroid gland makes and releases more thyroid hormone than your body needs. Your doctor may say you have an "overactive thyroid," or refer to the condition as "overactive thyroid disease."
Your thyroid gland is located in the front of your neck. Hormones released by the thyroid affect nearly every part of your body from yourbrain to your skin and muscles. They play a crucial role in controlling how your body uses energy, a process called metabolism. This includes how your heart beats and even how you burn calories.
Women are five to 10 times more likely to develop hyperthyroidism than men.
Causes of Hyperthyroidism The causes of hyperthyroidism include:
Graves' disease. The most common cause of hyperthyroidism is an autoimmune condition called Graves' disease. The body's immune system creates an antibody that causes the gland to make an excessive amount of thyroid hormone. Graves' disease runs in families, and usually affects younger women.
Thyroiditis. Thyroiditis is inflammation of the thyroid. A virus or problem with the immune system causes the gland to swell, leaking thyroid hormone into the bloodstream. There are several types of thyroiditis.
· Subacute: A sudden, painful form of thyroiditis of unknown cause. The thyroid usually heals on its own after a few months. The thyroid can become underactive for a while before it returns to normal.
· Postpartum: This type of thyroiditis affects women after pregnancy. One to two women out of every 10 women develop hyperthyroidism after having a baby. It usually lasts a month or two, followed by several months of underactive thyroid (hypothyroidism). In most women, the thyroid returns to normal.
· Silent: This type is similar to postpartum thyroiditis but is not related to pregnancy. The thyroid produces too much hormone but patients do not develop a painful thyroid gland. Some people may develop hypothyroidism afterwards.
Thyroid nodule. One or more lumps, or nodules, can grow in the thyroid gland, gradually increasing the gland's activity and the amount of thyroid hormone in your blood.
· If one nodule causes hyperthyroidism, it is called a single toxic nodule.
· If several nodules cause the thyroid to become overactive, the condition is called toxic multinodular goiter.
Excess iodine. You may also develop hyperthyroidism if you eat, drink, or are otherwise exposed to substances that contain a high amount of iodine. Iodine is used by the body to make thyroid hormone. Kelp or seaweed supplements and the medication amiodarone (Cordarone,Pacerone), once used to treat irregular heartbeats, are examples of medicines that contain a lot of iodine.
Thyroid medications. Taking too much thyroid hormone medication can wreak havoc on your thyroid gland and cause hyperthyroidism. If you have been prescribed thyroid replacement hormone (for hypothyroidism), never take an extra dose, even if you missed one, without first talking to your doctor.

What is Hyperthyroidism? The thyroid gland is a small butterfly-shaped gland located at the front of the neck below your Adam’s apple. It produces tetraiodothyronine (T4) and triiodothyronine (T3), two hormones which control how your cells use energy. The process by which cells use energy is called metabolism.
Hyperthyroidism occurs when too much T4 and/or T3 is produced. Proper diagnosis and treatment of the underlying cause relieves symptoms and prevents complications. Hyperthyroidism can run in families. Make sure to tell your doctor if there is a family history of the condition.
What Causes Hyperthyroidism? A variety of conditions can cause hyperthyroidism. Graves' disease (an autoimmune disorder) is the most common. It occurs more often in women and tends to run in families. In Graves' disease, antibodies stimulate the thyroid to secrete too much hormone. Other causes of hyperthyroidism include:
excess iodine (iodine is needed to make T4 and T3)
inflammation of the thyroid gland (thyroiditis causes T4 and T3 to leak out of the gland)
tumors of the ovaries or testes
benign tumors of the thyroid or pituitary gland
taking large amounts of tetraiodothyronine (through dietary supplements or medication)
What are the Symptoms of Hyperthyroidism? Symptoms are related to the effects of excess thyroid hormone. Thyroid hormones control metabolism, so excessive amounts of T4 or T3 cause a metabolic rate that is too high. This is called a hyper-metabolic state.
People with hyperthyroidism typically have rapid heart rates, weight loss, and heat intolerance. The thyroid gland can be visibly enlarged (goiter). You can also have elevated blood pressure, nervousness, and hand tremors. You may also sweat a lot, feel hungry and restless, and have difficulty concentrating. Your bowel movements may be more frequent and women may have irregular menstrual cycles. In Graves' disease, the eyes may appear quite prominent. This symptom is called exophthalmos. Other symptoms include:

hair loss
nausea and vomiting
breast development in men
Hyperthyroidism can also cause atrial fibrillation, a dangerous arrhythmia that can cause strokes. Congestive heart failure may also occur. Seek medical care immediately if you notice dizziness, shortness of breath, loss of consciousness, or fast irregular heart rate
Diagnosis
How is Hyperthyroidism Diagnosed? The first step is a complete history and physical exam. This can reveal common symptoms, such as weight loss, rapid pulse, elevated blood pressure, protruding eyes, and/or an enlarged thyroid gland (which can either appear either symmetrical or one-sided).Other tests may be performed to further evaluate your diagnosis. These include:
Cholesterol Level Test
Cholesterol levels vary with the metabolic rate. The metabolic rate is the rate at which cells use energy. In hyperthyroidism, cholesterol can be low due to the elevated metabolic rate.
T4 and T3RU (T3 Resin Uptake)Tests
These tests measure how much thyroid hormone is present in your blood.
TSH Level Test
TSH is a hormone produced by the hypothalamus that stimulates the thyroid gland to produce thyroid hormone. When your thyroid hormone levels are normal or high, TSH should not be elevated.
Triglyceride Level Test
Reasons for low triglyceride levels are the same as for low cholesterol levels.
Thyroid Scan and Uptake
This allows your doctor to see if your thyroid is overactive. It can also tell if the entire thyroid gland or just a single area of the gland is causing the overactivity.
Ultrasound
A doctor can use ultrasoung to measure the size of the entire thyroid gland, and any masses within it. An ultrasound allows doctors to know if the mass is solid or cystic.
Computed Tomography (CT) Scan or Magnetic Resonance Imaging (MRI):
A CT or MRI of the head is done if a pituitary tumor is suspected.
Treatment of Hyperthyroidism Medication Antithyroid medications and radioactive iodine are treatment mainstays. Antithyroid medications inhibit the synthesis of thyroid hormone and radioactive iodine effectively destroys the thyroid producing cells. Methimazole (Tapazole) is an example of an antithyroid medication. However, these medications can have severe side effects, such as low white blood cell count.
Surgery
Sometimes, a portion or all of your thyroid gland may have to be surgically removed. When this happens, thyroid hormone supplements must be taken to prevent hypothyroidism. Beta-blockers (propranolol) can help to control rapid pulse, sweating, anxiety, and blood pressure. Most people respond well to this treatment. Your doctor may refer you to an endocrinologist, a specialist in hyperthyroidism and other endocrine problems.
Treatment also is important to prevent thyroid storm or thyrotoxicosis and other complications. Thyroid storm is a sudden worsening of symptoms as a result of the release of large amounts of thyroid hormone. It can occur due to stress or infections.
Home Treatments
What you can do at Home to Improve Symptoms Getting the proper amount of calories, calcium, and sodium during and after treatment is important. A diet with too many calories can result in weight gain or obesity. Talk with your doctor and obtain healthy guidelines for your daily diet, nutritional supplements, and exercise.
Hyperthyroidism also can cause your bones to become thin (osteoporosis). Taking vitamin D and calcium supplements during and after treatment can help strengthen your bones. Make sure to ask your doctor about how much daily vitamin D and calcium is appropriate for you.
Long-Term Outlook for Hyperthyroidism The long-term outlook depends upon the cause. Some causes go away without treatment. Others, like Graves' disease, get worse over time. Complications of Graves' disease can be life threatening and affect quality of life for a long time.

If yours is overactive, a condition known as hyperthyroidism, your metabolism runs too fast, causing symptoms such as a rapid heartbeat, anxiety, shortness of breath and insomnia. In addition to any necessary medical treatment, a healthy diet, limited in certain foods, can help minimize your symptoms.
Food Allergens While foods affect people with hyperthyroidism differently, consuming foods your body is allergic to can worsen your symptoms. If you suspect that a food allergy is in play, the University of Maryland Medical Center recommends eliminating potential allergens, such as dairy products, soy, corn and chemical food additives, from your diet. You can also seek medical testing or engage in a guided elimination diet with your doctor or dietitian. If you test positive, avoid all foods containing the allergen.
Refined Grains When they're processed to create refined grains, grains lose valuable nutrients, such as B-vitamins, which play an important role in managing hyperthyroidism. As high-glycemic foods, refined grains can cause spikes and drops in your blood sugar levels, triggering or worsening mood swings. Common sources include energy drinks, coffee, black teas, chocolate and coffee-flavored ice cream and candies. Swap out black tea and coffee in your diet with herbal teas. Instead of sugary sodas, have sparkling or flat water with lemon. In place of coffee-flavored sweets, have a modest portion of dried fruit.
Sugary Sweets While modest amounts of sweets tend to be harmless, eating frequent or large amounts of added sugar can offset your blood sugar levels and worsen hyperthyroidism symptoms. To satisfy your sweet tooth, have fresh fruit, smoothies or baked sweet potatoes instead of sugary fare, such as candies, sweetened drinks, frosted desserts and pancake syrup.
For people with thyroid disease -- whether hypothyroid (underactive thyroid) or hyperthyroid (overactive thyroid), Hashimoto's disease or Graves' disease, or other conditions, there are some important things to know about foods and drinks, and their interaction with your health and medications. 1. About Goitrogenic Foods Goitrogens are substances -- occurring naturally in certain foods -- that can cause the thyroid gland to enlarge, which is called a goiter. Goitrogenic foods can also function like an antithyroid drug and actually slow down the thyroid and make it underactive (hypothyroidism.)
The main goitrogenic foods are cruciferous vegetables like broccoli, cauliflower, kale, and cabbage, among others, as well as soy foods.
If you still have a functional (or semi-functional thyroid gland), and are hypothyroid, you should be careful not to overconsume raw goitrogenic foods. If you are hyperthyroid, you may want to talk to a nutritional practitioner about incorporating more goitrogenic foods into your diet.
If you are hypothyroid, you don't need to avoid goitrogenic foods entirely . The enzymes involved in the formation of goitrogenic materials in plants can be at least partially destroyed by heat, allowing you to enjoy these foods in moderation if they are steamed or cooked.
2. Coconut Oil You may hear coconut oil recommended for thyroid patients , and while it can be a healthful option, it's not a cure-all or treatment for thyroid disease. It's just a thyroid-friendly option to replace other fats and oils in your diet.
3. Soy May Be a Problem Soy both acts as a goitrogen, and inhibits thyroid hormone absorption. Don't overconsume soy , especially processed and high-phytoestrogen forms of soy, like shakes, powders, soy milk, bars, and supplements. You may want to eliminate soy, or limit soy consumption to fermented forms, like tempeh, in small quantities as a condiment, and not as a primary protein replacement.
If you are hyperthyroid, you may want to talk to a nutritional practitioner about incorporating more soy into your diet.
4. Coffee And Thyroid Medication You should not take coffee until an hour after you've taken your thyroid hormone replacement medication . Otherwise, the coffee can affect absorption, and make your thyroid medication less effective.
(Note: if you absolutely must have both your thyroid medication and coffee at the same time, talk to your physician about the liquid, capsule form of levothyroxine, which is apparently not affected by coffee.)
5. Calcium-Fortified Orange Juice And Your Thyroid Medication You should not take calcium-fortified orange juice with your thyroid medication . Wait at least three to four hours after taking your thyroid medication before taking calcium-fortified juice, calcium supplements, or iron supplements, as they can interfere with your absorption of thyroid medication.
6. Iodized Salt In some areas of the world, iodized salt is an essential way to prevent iodine deficiency, cretinism and retardation due to iodine deficiency in pregnant women. In the U.S., however, many people have limited their salt intake, or stopped using iodized salt .
Keep in mind that about one-fourth of the U.S. population is now somewhat deficient in iodine, and that percentage appears to be on the rise again, after years of stable iodine levels (due to iodized salt intake.) You need enough iodine -- but not too much -- for the thyroid to function properly.
7. Celiac, Gluten and Wheat A subset of autoimmune thyroid patients have dietary-triggered autoimmunity, due to celiac disease , or a wheat/gluten intolerance. For these patients, going on a gluten-free diet may eliminate antibodies, and cause a remission of their autoimmune thyroid disease. Even for some patients who do not have celiac disease, going on a gluten-free diet may reduce antibodies, reduce bloating, and help with energy and weight loss.
8. High-Fiber Foods Many thyroid patients struggle with constipation, and extra weight. One of the key tactics that can help is increasing fiber intake, particularly from foods. Here is a list of high-fiber foods for thyroid patients .

If yours is overactive, a condition known as hyperthyroidism, your metabolism runs too fast, causing symptoms such as a rapid heartbeat, anxiety, shortness of breath and insomnia. In addition to any necessary medical treatment, a healthy diet, limited in certain foods, can help minimize your symptoms.
Food Allergens While foods affect people with hyperthyroidism differently, consuming foods your body is allergic to can worsen your symptoms. If you suspect that a food allergy is in play, the University of Maryland Medical Center recommends eliminating potential allergens, such as dairy products, soy, corn and chemical food additives, from your diet. You can also seek medical testing or engage in a guided elimination diet with your doctor or dietitian. If you test positive, avoid all foods containing the allergen.
Refined Grains When they're processed to create refined grains, grains lose valuable nutrients, such as B-vitamins, which play an important role in managing hyperthyroidism. As high-glycemic foods, refined grains can cause spikes and drops in your blood sugar levels, triggering or worsening mood swings. To avoid these effects and reap plentiful nutrients in the process, replace refined grains, such as white flour and instant rice, in your diet, with whole grains, such as quinoa, oats and wild rice. Common sources include energy drinks, coffee, black teas, chocolate and coffee-flavored ice cream and candies. Swap out black tea and coffee in your diet with herbal teas. Instead of sugary sodas, have sparkling or flat water with lemon. In place of coffee-flavored sweets, have a modest portion of dried fruit.
Sugary Sweets While modest amounts of sweets tend to be harmless, eating frequent or large amounts of added sugar can offset your blood sugar levels and worsen hyperthyroidism symptoms. To satisfy your sweet tooth, have fresh fruit, smoothies or baked sweet potatoes instead of sugary fare, such as candies, sweetened drinks, frosted desserts and pancake syrup.

1. About Goitrogenic Foods Goitrogens are substances -- occurring naturally in certain foods -- that can cause the thyroid gland to enlarge, which is called a goiter. Goitrogenic foods can also function like an antithyroid drug and actually slow down the thyroid and make it underactive (hypothyroidism.)
The main goitrogenic foods are cruciferous vegetables like broccoli, cauliflower, kale, and cabbage, among others, as well as soy foods.
If you still have a functional (or semi-functional thyroid gland), and are hypothyroid, you should be careful not to overconsume raw goitrogenic foods. If you are hyperthyroid, you may want to talk to a nutritional practitioner about incorporating more goitrogenic foods into your diet.
If you are hypothyroid, you don't need to avoid goitrogenic foods entirely . The enzymes involved in the formation of goitrogenic materials in plants can be at least partially destroyed by heat, allowing you to enjoy these foods in moderation if they are steamed or cooked.
2. Coconut Oil You may hear coconut oil recommended for thyroid patients , and while it can be a healthful option, it's not a cure-all or treatment for thyroid disease. It's just a thyroid-friendly option to replace other fats and oils in your diet.
3. Soy May Be a Problem Soy both acts as a goitrogen, and inhibits thyroid hormone absorption. Don't overconsume soy , especially processed and high-phytoestrogen forms of soy, like shakes, powders, soy milk, bars, and supplements. You may want to eliminate soy, or limit soy consumption to fermented forms, like tempeh, in small quantities as a condiment, and not as a primary protein replacement.
If you are hyperthyroid, you may want to talk to a nutritional practitioner about incorporating more soy into your diet.
4. Coffee And Thyroid Medication You should not take coffee until an hour after you've taken your thyroid hormone replacement medication . Otherwise, the coffee can affect absorption, and make your thyroid medication less effective.
(Note: if you absolutely must have both your thyroid medication and coffee at the same time, talk to your physician about the liquid, capsule form of levothyroxine, which is apparently not affected by coffee.)
5. Calcium-Fortified Orange Juice And Your Thyroid Medication You should not take calcium-fortified orange juice with your thyroid medication . Wait at least three to four hours after taking your thyroid medication before taking calcium-fortified juice, calcium supplements, or iron supplements, as they can interfere with your absorption of thyroid medication.
6. Iodized Salt In some areas of the world, iodized salt is an essential way to prevent iodine deficiency, cretinism and retardation due to iodine deficiency in pregnant women. In the U.S., however, many people have limited their salt intake, or stopped using iodized salt .
Keep in mind that about one-fourth of the U.S. population is now somewhat deficient in iodine, and that percentage appears to be on the rise again, after years of stable iodine levels (due to iodized salt intake.) You need enough iodine -- but not too much -- for the thyroid to function properly.
7. Celiac, Gluten and Wheat A subset of autoimmune thyroid patients have dietary-triggered autoimmunity, due to celiac disease , or a wheat/gluten intolerance. For these patients, going on a gluten-free diet may eliminate antibodies, and cause a remission of their autoimmune thyroid disease. Even for some patients who do not have celiac disease, going on a gluten-free diet may reduce antibodies, reduce bloating, and help with energy and weight loss.
8. High-Fiber Foods Many thyroid patients struggle with constipation, and extra weight. One of the key tactics that can help is increasing fiber intake, particularly from foods. Here is a list of high-fiber foods for thyroid patients .
Keep in mind, however, that if you switch to a high-fiber diet, you should get your thyroid rechecked in eight to twelve weeks to see if you need a dosage readjustment, as fiber can affect absorption of medication.
